Q 1. Describe briefly the Cause and Effect Diagram with example.
Answer : When you have a serious problem, it's important to explore all of the things that could cause it, before you start to think about a solution.
That way you can solve the problem completely, first time round, rather than just addressing part of it and having the problem run on and on





2. (a) What is the meaning of a ‘process’? Explain with an example.

 (b) Write a brief note on SIPOC.
Answer : A SIPOC diagram is a type of process map typically used in Lean Six Sigma projects to identify the primary elements of a process. It provides a macro view that brings together Suppliers, Inputs, Process, Outputs, and Customers. This article provides a simple and effective method for building a SIPOC with your team.

Answer :In the Six Sigma quality methodology, process performance is reported to the organization as a sigma level. The higher the sigma level, the better the process is performing.
